Important information on your appointment for Canada

 

Beginning on December 15, 2016, no appointments will be made for Upfront medical examination for Sponsorship applicants (Wife and dependent child). For more information visit the “Family Sponsorship” section of the Migration Canada website.

The following categories: Students, Workers, Visitors and Express Entry Applications The Upfront medical examination can still be provided by our clinic.

    PAYMENT:

    • Visa or MasterCard credit or debit cards.
    • Payments in cash or by the bank (the account number will be provided once the medical examination is over).
    • No payments in dollars will be accepted.
    • Regardless of whether your appointment is in Cd. Juarez you can take the medical exams with us as they are valid.
    • We suggest you schedule your appointment 1 month in advance of your appointment at the Ciudad Juárez Consulate.
     

    IMPORTANT: The use of false vaccination documents represents the risk of significantly delaying the whole process, visa forfeiture, and exposes you to be charged with fraud; do not compromise the process of obtaining visa, let us know immediately if any of your documents is false.

    Before going to your appointment, please consider the following:

    • To be sure your visa appointment is at the Embassy of Mexico City.
    • We suggest you schedule your appointment with 1 month in advance regarding your appointment at the Consulate in Ciudad Juarez.

    Requirements for medical examination:

    Every applicant for Canadian visa must present ONE of the following forms as appropriate:
    • Format "Appendix C" previously filled in for Sponsorship visa applicants. Fill and print "Appendix C" format by. clicking here.

    • Applicants for other types of visas issued by the Department of Immigration Canada must print their "Medical Report".
    • Current, original and copy of the first and last page of Passport, otherwise bring any photo ID.
    • Birth Certificate (original and copy).
    • Adult companion for children under 15 years old must bring photo ID (original and copy).
    • Children under 15 years old can have breakfast before their medical examination.

    •     Newborns and children up to 3 years old of age must submit:

      Immunization records and copies of all pages containing information including personal data and vaccines.

      Birth Certificate issued by the Ministry of Health and Welfare.

      Copy of the result of the metabolic and auditory sieve.

      No fasting is required.

    • Consider that if you do not respect the conditions stipulated, we cannot guarantee delivery time.


    All applicants from 6 to 14 years old must present:
    • The same requirements as the ones of 15 and older, but for these ones is not necessary to fast.


    All applicants of 4 and 5 years old must present:

    • Appendix C Format previously elaborated for visa applicants with sponsorship. Fill and print "Appendix Format C " click here

    • Format A (Medical Report) for applicants to other kind of visa issued by the Canadian Immigration Department.

    • Present original and a photocopy of the first and last page of the current passport. If you do not have it report it at the time you are making your appointment.
    • Photocopy and original of the birth certificate.
    • Photocopy and original of the official ID from the adult companion.
    • National Vaccination Card original and photocopy of all the pages that contain any information.
    • In case there were any medical records it is necessary that you present a doctor´s evidence on letterhead in English.
    • If you are under a medical treatment, or if you have had a big operation during the last 5 years please show a doctor´s evidence on letterhead and in English and a copy of the pathology report.
    • The urine sample will be collected in our facilities to the 5 year-old applicants and is necessary to be the first one of the day.
    • It is not necessary to fast.


    All applicants under 3 years of age or younger must present:

    One of the following formats as appropriate.

    • Format A (Medical Report) for applicants to other kind of visa issued by the Canadian Immigration Department.

    • Present original and a photocopy of the first and last page of the current passport. If you do not have it report it at the time you are making your appointment.
    • Photocopy and original of the birth certificate.
    • Photocopy and original of the official ID from the adult companion.
    • National Vaccination Card original and photocopy of all the pages that contain any information.
    • It is not necessary to fast.
    • The birth certificate should have the following:
    • Type of delivery: Vaginal delivery or cesarean
      Apgar and Silverman
      Sieve: (Metabolic and/or auditory)
      Head Circumference

    • In case there were any medical records it is necessary that you present a doctor´s evidence on letterhead in English.
    • If you are under medical treatment, or was subjected to any more please submit proof of your doctor in English operation, letterhead and copy of pathology report.
