Requirements for your medical examination:
We suggest you contact us 2 weeks prior to your desired appointment date.
Appointment form issued by the Canadian Embassy, MEDICAL REPORT (contains IME, UCI or EMR).
Original and copy of valid passport; if you do not have a passport, original and copy of birth certificate.
In the case of minors under 16 years of age, a copy of the identification of the parent or guardian.
Children under 5 years of age must present: Original official vaccination card, neonatal screening,
Hospital discharge certificate and a copy of each.
IMPORTANT: If you have received one or more Covid-19 vaccines, please submit your COVID-19 vaccination
certificate (Updated) and proof of vaccination.
Before going to your appointment, consider:
6-hour fast. (Non-liquids and Non-solids) for applicants over 15 years old.
For applicants over 5 years old, the first urine of the day will be collected at our facilities.. (It is recommended to go to the
bathroom for the last time at 03:00 am).
Please arrive 5 minutes before your appointment time.
Please bring your personal email address and contact phone number.
For women NOT SHOWING UP DURING THE MENSTRUAL CYCLE.
If you wear glasses, you must bring them.
Come in comfortable clothing, without accessories.
Come to your appointment without companions (except minors under 18 years of age and people with disabilities).
If you have had surgery in the last 5 years or suffer from chronic illnesses, please bring a medical summary that is less than 3 months old
(with diagnosis, treatment, prognosis, on letterhead and in English or French), If you suffer from high blood pressure,
(presión alta) please bring a recent electrocardiogram (less than 3 months old).
Requirements for taking photos:
Bare neck, forehead and ears uncovered, hair tied back, no scarves, accessories and/or earrings.
ATTENTION:Beginning on December 15, 2016, no appointments will be made
for Upfront medical examination for Sponsorship applicants (Wife and dependent child).
